摘要:
目的 分析血嗜酸性粒细胞(EOS)、总IgE与儿童哮喘严重程度和肺功能的相关性。方法 选择2018年1月至2021年1月我院收治并确诊的55例支气管哮喘患儿,根据病情严重程度分为轻度组18例、中度组22例、重度组15例。对比不同病情严重程度组儿童血清EOS、总IgE、FeNO及肺功能第1秒用力呼气容积占预测值百分比(FEV1% pred)、呼气峰值流速(PEF)和用力肺活量(FVC)水平变化; 观察给予吸入性糖皮质激素(ICS)治疗前后,哮喘组儿童上述指标变化情况; 采用Spearman秩相关和Pearson 相关性分析血清EOS、总IgE与儿童哮喘FeNO、病情严重程度及肺功能的相关性。结果 哮喘组儿童FeNO及血EOS%、总IgE水平升高,肺功能指标FEV1% pred、PEF、FVC明显降低(P<0.05); 重度哮喘组上述指标高于/低于中度组,中度组高于/低于轻度组(P<0.05)。ICS治疗后,哮喘组儿童FeNO及血EOS%、总IgE水平较治疗前降低,肺功能指标升高,治疗前后差异有统计学意义(P<0.05)。儿童哮喘血EOS%和总IgE水平呈正相关(P<0.05); 血EOS%、总IgE与FeNO和病情严重程度呈正相关,与FEV1% pred呈负相关(P<0.05),与PEF和FVC未见显著相关性(P>0.05)。结论 儿童哮喘血EOS和总IgE水平明显升高,与FeNO、病情严重程度及肺功能相关,监测血EOS和总IgE水平变化可为儿童哮喘的临床诊治和病情监测提供参考。
Abstract:
Objective To analyze the correlation between blood eosinophils(EOS), total IgE and asthma severity and lung function in children. Methods A total of 55 children with bronchial asthma admitted and diagnosed in our hospital from January 2018 to January 2021 were prospectively selected and divided into mild group 18 cases, moderate group 22 cases and severe group 15 cases according to the severity of their disease.The changes of serum EOS, total IgE, FeNO, the percentage of forced expiratory volume in the first second of lung function to predicted value(FEV1% pred), peak expiratory flow rate(PEF)and forced vital capacity(FVC)in children with different severity of the disease. To observe the changes of the above indexes in asthma group before and after inhaled glucocorticoid(ICS)treatment. Spearman rank correlation and Pearson correlation were used to analyze the correlation between serum EOS, total IgE and FeNO, severity of the disease and lung function in children with asthma. Results The levels of FeNO, EOS% and total IgE in asthmatic group were significantly increased, and the pulmonary function indexes FEV1% pred, PEF and FVC were significantly decreased(P<0.05); The above indexes in severe asthma group were higher/lower than those in moderate group, and those in moderate group were higher/lower than those in mild group(P<0.05). After ICS treatment, FeNO, serum EOS% and total IgE levels of children in asthma group decreased compared with before treatment, while pulmonary function indexes increased, with statistical significance before and after treatment(P<0.05). There was a positive correlation between EOS% and total IgE level in asthmatic children(P<0.05). Serum EOS% and total IgE were positively correlated with FeNO and disease severity, but negatively correlated with FEV1% pred(P<0.05), but had no significant correlation with PEF and FVC(P>0.05). Conclusion The serum EOS and total IgE levels were significantly increased in children with asthma, which were significantly correlated with FeNO, disease severity and lung function. Monitoring the changes of serum EOS and total IgE levels could provide valuable reference for the clinical diagnosis, treatment and disease monitoring of children with asthma.
