摘要:

目的 分析药物依从性对慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)患者焦虑、抑郁与生活质量的影响并建立结构方程模型,为提高COPD生活质量提供干预依据。方法 选取2018年2月至2020年10月我院门诊和住院纳入的300例COPD患者为对象,采用医院焦虑抑郁量表(HADS)、COPD评估(CAT)、 Morisky 药物依从性量表(MMAS-4)对COPD患者进行测评,运用结构方程模型软件AMOS 24.0进行分析。结果 300例COPD患者中焦虑115例(38.33%),抑郁110例(36.67%),同时患有焦虑和抑郁87例(29.00%)。模型数据拟合结果,χ²/df=1.011<3,GFI=0.938>0.9,AGFI=0.924>0.9,CFI=0.999>0.9,NFI=0.931>0.9,IFI=0.999>0.9,RMSER=0.006<0.1, 焦虑抑郁是生活质量的影响因素,总效应分别为 0.338、0.255,患者药物依从性是焦虑抑郁对CAT评分的中介变量。结论 COPD患者焦虑抑郁、药物依从性和生活质量密切相关,药物依从性在焦虑抑郁对CAT评分影响中产生中介作用,COPD患者焦虑抑郁负性情绪直接或间接地影响了其药物依从性与生活质量,重视患者负性情绪的评估,及时开展针对性的心理干预措施。

Abstract:

Objective Explore the effects of medication compliance on anxiety, depression and quality of life in patients with chronic obstructive pulmonary disease(COPD)and establish a structural equation model, so as to provide an intervention basis for improving the quality of life in patients with COPD. Methods From February 2018 to October 2020, 300 patients with chronic obstructive pulmonary disease in the Second Affiliated Hospital of Army Medical University were selected as the research objects. Hospital Anxiety and Depression Scale(HADS), chronic obstructive pulmonary Disease Assessment(CAT)and Morisky Medication Adherence Scale(MMAS-4)were used to evaluate patients with chronic obstructive pulmonary disease, and structural equation modeling software AMOS 24.0 was used for analysis. Results In 300 COPD patients, 115 cases(38.33%)were anxious, 110 cases(36.67%)were depressed, and 87 cases(29.00%)were both anxious and depressed. The model data fit results, χ²/df=1.011<3,GFI=0.938>0.9, AGFI=0.924>0.9, CFI=0.999>0.9, NFI=0.931>0.9, IFI=0.999>0.9, RMSER=0.006<0.1, anxiety and depression were the influencing factors of quality of life, the total effect was 0.338, 0.255, respectively. Patient medication compliance is a mediating variable between anxiety and depression and CAT score. Conclusions Anxiety and depression, medication adherence and quality of life are closely related in COPD patients. Medication adherence plays a mediating role in the influence of anxiety and depression on CAT score. The negative emotions of anxiety and depression in patients with chronic obstructive pulmonary disease directly or indirectly affect their medication compliance and quality of life. It is suggested that attention should be paid to the evaluation of patients'negative emotions and timely carry out targeted psychological intervention measures.
