摘要:
目的 分析经鼻导管高流量氧疗在机械通气患者撤机后的应用效果。方法 在线检索PubMed、Cochrane Library、EMBASE、CINAHL、MEDLINE、中国知网(CNKI)、中国生物医学文献数据库、维普数据库、万方数据库,关于经鼻导管高流量氧疗在机械通气患者撤机后影响的随机对照试验; 查阅文献资料进行提取,应用Cochrane评价手册对文献质量进行分析,对符合质量标准的随机对照实验应用RevMan5.3软件进行Meta分析。 结果 共纳入12篇研究,共包括1 066例患者。Meta分析结果显示:与常规氧疗组相比,ICU机械通气患者撤机后应用经鼻导管高流量氧疗,不仅可提高人工气道患者的湿化效果[Z=2.65,RR=1.51,95%CI(1.11,2.05),P=0.008],而且能提高患者脱机成功率[RR=1.31,95%CI(1.17,1.47),Z=4.70,P<0.001],降低人工气道患者再插管率[RR=0.26,95%CI(0.15,0.44),Z=5.00,P<0.001],对降低肺部感染有积极作用[RR=0.26,95%CI(0.15,0.44),Z=5.00,P<0.001]。结论 ICU机械通气患者撤机后应用经鼻高流量氧疗有助于改善患者预后。
Abstract:
Objective To systematically evaluate the effect of high-flow nasal cannula therapy(HFNC)after weaning patients with mechanical ventilation. Methods Pub Med, Cochrane Library, EMBASE, CINAHL, MEDLINE, CNKI, Chinese biomedical literature database, VIP database and Wanfang database were searched online. Randomized controlled trials(RCTs)on the effect of high-flow nasal cannula therapy on mechanical ventilation patients after weaning were conducted; Two researchers extracted the literature, evaluated the literature quality with Cochrane evaluator manual, and RevMan5.3 software was used for meta-analysis. Results a total of 12 studies were included, including 1 066 patients. The results of meta-analysis showed that compared with the conventional oxygen inhalation group, the application of high-flow nasal cannula therapy after weaning in ICU mechanical ventilation patients could not only improve the humidification effect of patients with artificial airway [Z=2.65, RR=1.51, 95%CI(1.11, 2.05), P=0.008], but also improve the offline power of patients [RR=1.31, 95%CI(1.17, 1.47), Z=4.70, P<0.001], It can effectively reduce the re intubation rate of patients with artificial airway [RR=0.26, 95%CI(0.15, 0.44), Z=5.00, P<0.001]. Conclusion The application of high-flow nasal cannula therapy after mechanical ventilation in ICU is helpful to improve the prognosis of patients, which is worthy of clinical promotion.
