摘要:
目的 分析依诺肝素治疗高血压脑出血并肺栓塞的疗效和安全性。方法 回顾2017年5月至2022年5月我院收治的65例高血压脑出血并肺栓塞患者的临床资料,按照治疗方式分为观察组31例(依诺肝素治疗)和对照组34例(普通肝素治疗),比较患者的临床疗效、血气指标、心肌标志物、肺功能、凝血功能和不良反应。结果 观察组治疗后在显效率和总有效率方面高于对照组,差异有统计学意义(P<0.05); 氧分压(PaO2)、氧合指数(OI)高于治疗前,二氧化碳分压(PaCO2)低于治疗前; 观察组在PaO2、OI方面高于对照组,PaCO2数值低于对照组(P<0.05); 肌酸激酶同工酶(CK-MB)、N末端脑钠肽前体(NT-proBNP)低于治疗前,观察组低于对照组(P<0.05); 第1秒最大的呼吸效率(FEV1/FVC)、最大呼气流量(PEF)高于治疗前,观察组高于对照组(P<0.05); 纤维蛋白原(FIB)、凝血酶时间(TT)、D-二聚体(D-D)水平与治疗前相比无统计学差异(P>0.05)。治疗后不良反应发生率无统计学差异(P>0.05)。结论 依诺肝素治疗高血压脑出血并肺栓塞的疗效好,安全。
