摘要:

目的 分析早期个性化心肺运动康复(cardiopulmonary rehabilitation exercise training, CPET)预防冠状动脉搭桥术(coronary artery bypass grafting, CABG)术后肺部并发症的应用效果。方法 选择2022年1月至2024年1月我院接受CABG手术患者53例为对象, 行早期个性化CPET 28例为观察组,常规运动康复25例为对照组。比较两组术后肺部并发症发生率,治疗4周后心功能指标、肺功能指标、不良情绪及生活质量评分。结果 观察组治疗后,不良反应发生2例(7.2%); 对照组发生9例(36.0%),观察组不良反应发生率低于对照组(P<0.05)。治疗4周后,观察组血红蛋白浓度(hemoglobin, Hb)(121.85±2.12)g/L,血小板计数(platelet count, PLT)(193.27±1.59)×10⁹/L,甘油三酯(triglyceride, TG)(3.33±0.04)mmol/L,血清总胆固醇(total cholesterol, TC)(1.03±1.08)mmol/L,低密度脂蛋白(low densith lipoprotein, LDL)(2.01±0.23)mmol/L,高密度脂蛋白(high density lipoprotein, HDL)(1.95±0.43)mmol/L优于对照组(140.75±1.21)g/L、(169.90±2.24)×10⁹/L、(4.39±0.33)mmol/L、(1.87±1.35)mmol/L、(3.45±1.65)mmol/L、(1.42±0.04)mmol/L(P<0.05); 观察组心率(81.03±2.06)次/min、左心房容积(30.42±1.10)ml、氧脉搏(VO₂/HR)(12.25±1.20)次/min、左心室血流传播速度(46.14±1.23)cm/s、第1秒用力呼气容积占预计值百分比(FEV₁/FVC)(80.67±1.44)% 、峰值摄氧量(Peak VO₂)(19.92±2.22)ml/min、峰值分钟通气量(minute ventilation volume, MVV)(42.21±2.28)L/min、峰值脉氧饱和度(percutaneous arterial oxygen saturation, SpO₂)(94.46±1.03)%优于对照组(82.60±2.16)次/min、(43.24±1.16)ml、(10.68±0.85)次/min、(43.24±1.16)cm/s、(77.92±1.25)%、(18.24±1.61)ml/min、(40.44±1.19)L/min、(92.48±0.65)%(P<0.05); 治疗后,观察组WHOQOL评分(78.00±0.65)分 高于对照组(74.00±1.50)分(P<0.05); 观察组HADS评分(15.53±0.79)分低于对照组(20.08±2.02)分(P<0.005)。结论 早期个性化CPET有效降低CABG术后肺部并发症发生率,改善心功能及肺功能,缓解不良情绪,提高生活质量。
