摘要:
目的 分析个体化肺康复联合情绪调节干预对慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary disease, AECOPD)并静脉血栓栓塞(venous thrombo embolism, VTE)高危患者生活质量和预后的影响。方法 选择2022年12月至2023年12月我院收治的64例AECOPD并VTE高危患者为对象,实施常规临床管理30例为对照组,在常规管理上实施个体化肺康复和情绪调节干预34例为观察组。比较治疗前后两组抑郁自评量表(self-rating depression scale, SDS)、焦虑自评量表(self-rating anxiety scale, SAS)、圣乔治呼吸问卷(St George's respiratory questionnaire, SGRQ)评分,记录两组治疗效果,随访干预6个月后急性加重、再住院情况。结果 观察组治疗后SAS(51.56±1.76)分、SDS(42.56±2.36)分低于对照组(56.47±2.36)分、(48.34±2.89)分,(P<0.05); 观察组SGRQ症状评分(45.3±9.87)分、活动评分(51.11±9.94)分、影响评分(48.66±11.18)分低于对照组(48.4±7.47)分、(56.38±11.37)分、(54.32±14.56)分(P<0.05); 观察组临床有效27例(84.38%)高于对照组(73.33%); 观察组急性加重率9.38%、VTE发生率6.26%低于对照组急性加重率30.00%、33.33%(P<0.05); 观察组再住院率6.26%低于对照组16.70%(P<0.05)。结论 个体化肺康复联合情绪调节改善AECOPD患者不良情绪,提高生存质量、改善预后,具有临床意义。
