摘要:
目的 分析肿瘤坏死因子样细胞弱凋亡因子(tumor necrosis factor-like cell weak apoptosis factor, TWEAK)在诱导痰中的表达以及与哮喘严重程度的关联性。方法 收集2022年3月至2022年9月我院收治的50例哮喘患者为对象,使用酶联免疫吸附法测定诱导痰TWEAK值。根据TWEAK中位值分为TWEAK高表达组(≥0.70 ng/ml)和TWEAK低表达组(<0.70 ng/ml),每组25例。使用Spearman分析TWEAK与肺功能指标、炎症指标和M1型巨噬细胞的相关性。结果 TWEAK高表达组干扰素(interferon, IFN)-γ 0.80(0.24,2.07)pg/ml低于低表达组3.07(0.61,17.31)pg/ml(P<0.05); 高表达组白细胞介素(interleukin, IL)-17A 0.35(0.07,1.22)pg/ml、CD45+/CD68+巨噬细胞% 5.98(3.36,8.02)、M1型(CD11c+/CD86+)巨噬细胞% 5.44(1.59,8.44)高于TWEAK低表达组0.11(0.03,0.23)pg/ml、3.03(1.60,5.75)、2.45(1.52,4.47)(P<0.05)。经Spearman分析,诱导痰TWEAK与 1 秒用力呼气容积(FEV1)占预计值的百分比(rs=-0.476)、FEV1/用力肺活量(rs=-0.394)、诱导痰IFN-γ(rs=-0.346)呈显著负相关(P<0.05)。诱导痰TWEAK与肿瘤细胞坏死因子(tumor necrosis factor,TNF)-α(rs=0.315)、IL-6(rs=0.286)、IL-17A(rs=0.320)、M1型(CD11c+/CD86+)巨噬细胞%(rs=0.470)、中性粒细胞%(rs=0.293)以及外周血中性粒细胞%(rs=0.351)呈显著正相关(P<0.05)。有过敏家族史的患者诱导痰TWEAK表达显著低于无过敏家族史(P<0.05)。结论 诱导痰TWEAK过表达与哮喘患者炎症进展和疾病发作存在相关性,诱导痰中TWEAK可能是哮喘严重程度的标志物。
Abstract:
Objective To investigate the expression of tumor necrosis factor-like cell weak apoptosis factor(TWEAK)in induced sputum and its association with asthma severity. Methods A total of 50 asthma patients were recruited from March 2022 to September 2022, and induced TWEAK in sputum were determined using enzyme-linked immunosorbent assay. According to the TWEAK value, asthma patients were divided into TWEAK high expression group(≥0.70 ng/ml)and TWEAK low expression group(<0.70 ng/ml), with 25 cases in each group. Spearman was used to analyze the correlation between TWEAK and lung function indexes, inflammation markers and M1 macrophages. Results The interferon(IFN)-γ in the TWEAK high expression group were significantly lower than those in the low expression group(P<0.05), and the percentages of interleukin(IL)-17A, CD45+/CD68+ macrophages%, M1 type(CD11c+/CD86+)macrophages% in the TWEAK high expression group were significantly higher than those in the low expression group(P<0.05). According to Spearman analysis, induced sputum TWEAK was significantly negatively correlated with the percentage of forced expiratory volume(FEV1)in 1 second(rs=-0.476), FEV1/forced vital capacity(rs=-0.394), and induced sputum IFN-γ(rs=-0.346)(P<0.05). The induced sputum TWEAK was significantly positively correlated with tumor cell necrosis factor-α(rs=0.315), IL-6(rs=0.286), IL-17A(rs=0.320), M1 type(CD11c+/CD86+)macrophage%(rs=0.470), neutrophil%(rs=0.293)and peripheral blood neutrophil%(rs=0.351)(P<0.05). The expression of induced sputum TWEAK in patients with a family history of allergies was significantly lower than that in patients without a family history of allergies(P<0.05). Conclusion There was a significant correlation between induced sputum TWEAK overexpression and inflammation progression and disease flare-up in asthma patients, and induced TWEAK in sputum may be a marker of asthma severity.
